If employed, your company’s HR Department will process it for you. Accomplish SSS Form ISL-101 or any of the requirements listed below if you are filing by yourself.

    Professional Regulation Commission (PRC) ID card
    Seaman’s Book
    Driver’s License
    RS-1 (Self-employed Data Record)

    Alien Certificate of Registration
    Birth Certificate
    Health or Medical Card
    Major credit card
    Marriage contract
    Valid NBI Clearance
    Transcript of Records
    TIN Card

The current program of SSS Loan Application in the Philippines (only those affected by retrenchment) please proceed or call the Department of Labor and Employment and secure a certificate of your previous employment. Those with standing SSS Loans are not eligible to apply.
